We are seeking a compassionate and skilled Nurse Practitioner to provide high-quality care to residents in skilled nursing facilities. The ideal candidate will excel in managing chronic and acute conditions, collaborating with interdisciplinary teams, and enhancing the quality of life for elderly and rehabilitating patients in a dynamic healthcare setting.

Key Responsibilities:

Perform comprehensive assessments, diagnose, and develop treatment plans for residents with complex medical needs.
Manage chronic conditions (e.g., diabetes, hypertension, COPD) and acute illnesses in collaboration with physicians.
Order and interpret diagnostic tests, prescribe medications, and adjust care plans as needed.
Conduct regular rounds, providing bedside care and follow-up in skilled nursing facilities.
Educate residents, families, and staff on health management and preventive care.
Document patient care accurately in electronic health records, ensuring compliance with state and federal regulations.
Participate in care coordination meetings with facility staff, therapists, and social workers.
Master’s degree in Nursing and current Nurse Practitioner certification (e.g., ANCC or AANP).
Active [State] NP license and DEA registration (or eligibility).
Minimum of 1-2 years of clinical experience, preferably in geriatrics or long-term care.
Knowledge of skilled nursing facility regulations and Medicare/Medicaid requirements.
Strong interpersonal skills to build trust with residents, families, and staff.
Ability to work independently and as part of a team in a fast-paced environment.

Physical Requirements:
Ability to stand and walk for extended periods during rounds.
Capacity to lift up to 20 pounds occasionally.
Manual dexterity for clinical tasks and documentation.
