We are seeking a dedicated Home Health Registered Nurse to provide skilled nursing care to patients in their homes. The ideal candidate will deliver compassionate, patient-centered care, manage diverse health conditions, and promote independence while working independently in a community-based setting.

Key Responsibilities:

Assess patient health status, develop, and implement individualized care plans.
Administer medications, wound care, IV therapy, and other treatments as prescribed.
Monitor and document patient progress, reporting changes to physicians and care teams.
Educate patients and families on disease management, medication, and self-care techniques.
Coordinate with home health aides, therapists, and social workers to ensure comprehensive care.
Maintain accurate electronic records in compliance with Medicare, Medicaid, and state regulations.
Ensure patient safety and infection control in home environments.

Associate’s or Bachelor’s degree in Nursing and current RN license in NYS.
Minimum of 1-2 years of nursing experience, preferably in home health or community care.
BLS certification; knowledge of home health regulations and reimbursement processes.
Strong assessment, communication, and organizational skills.
Reliable transportation, valid driver’s license, and auto insurance.
Ability to work independently with a diverse patient population.

Physical Requirements:
Ability to lift up to 50 pounds and assist with patient mobility.
Frequent standing, walking, and driving between patient homes.
Manual dexterity for clinical tasks and documentation.
